Providence Physical Therapist in Mission Hills, California

Description:

Providence is calling a Physical Therapist (On Call/ Day Shift ) to Providence Facey Medical Foundation in Mission Hills, CA.

We are seeking a Physical Therapist who will provide delivery of high quality patient care at the direction of medical physician in accordance with individual diagnosis, medical group policy and standards of patient care. The primary function is to provide assessment, treatment, planning, implementation, education, communication and demonstration of leadership to address the needs of patients and their families, as well as to foster a positive working relationship among professionals and ancillary staff. May be caring for infants, children, adolescents, adults and/or senior patients and will have the knowledge and skills to care for the physical and developmental needs of patients.

In this position you will:

  • Perform patient assessments and ensures the efficient delivery of high quality patient care:

  • Deliver upon the service expectations of both our patients and fellow staff members by listening to their needs; engaging in positive interactions; and following through on promises made in a thoughtful, efficient, timely and courteous manner so that their total outcome is better than expected

  • Respect the dignity, confidentiality and privacy of patients

  • Work in a safe manner, adhering to general safety precautions and standards. Report any unsafe conditions to their supervisor and/or the safety hotline

  • Provide direct patient care, evaluate functional needs and outcomes, consult with other specialists as needed

  • Reassess patient’s functional needs, and adjust care plan as indicated to ensure optimal patient outcome

  • Design and implement a plan of care for the patient and significant others based on a thorough assessment

  • Conduct therapy practice within defined scope of standards of care

  • Direct the practice of support staff in order to achieve the patient’s functional goals

  • Provide accurate, complete, legible, timely documents of evaluations, treatment goals, and plans. Regularly update documentation and maintains all required prescriptions and signatures

  • Follow correct procedures for the handling and disposing of biohazardous infectious waste

  • Comply with all applicable infection control and safety procedures

  • Demonstrate awareness of current state, federal and local laws governing the delivery of care:

  • Use appropriate reporting mechanisms as required by state laws

  • Respect the dignity, confidentiality and privacy of patients

  • Obtain appropriate authorization, consents for treatment

  • Provide open and timely communication with patients, families and their significant others:

  • Provide sufficient information to allow patients and significant others to participate in the patient's care

  • Provide educational resources to the patient/family as appropriate

  • Promote and contributes to a positive, problem-solving environment

  • Demonstrate commitment to ongoing education and training:

  • Enhance professional growth and development of self and through participation in educational programs, in-service meetings and workshops

  • Maintain current American Heart Association BLS for Health Care Providers and other appropriate licensure/certifications

Qualifications:

Required qualifications for this position include :

  • Bachelor's Degree in Physical Therapy or graduate of APTA approved physical therapy program

  • Current CA Physical Therapist license

  • American Heart Association BLS for Health Care Providers

  • 1 year clinical practice as a Physical Therapist
