We are seeking a Clinical Pharmacist Specialist who will be reporting to the Director of Pharmacy, this position is responsible for providing and coordinating pharmaceutical care services and collaborative drug therapy management for patients. The Clinical Pharmacist Specialist will serve as a resource to the patients, physicians, pharmacists, nurses and other members of the health care team. The clinical pharmacist specialist: 1) assures safe and effective medication use; 2) demonstrates proficiency in delivering care to assigned age specific patient population (e.g., infants, adolescents, adults, geriatrics, and pediatrics); 3) participates in related continuing education; and 4) manages drug therapy protocols as a part of an interdisciplinary team.
In this position you will:
Implement and maintain established department, unit, and hospital policies, procedures, and objectives as well as improving organization performance program, safety, environmental, and infection control policies.
Demonstrate excellent customer service and positive interpersonal relationships with physicians, staff, and patients/families.
Actively participate in daily patient care to identify patients for therapeutic monitoring and to discuss potential interventions.
Provide comprehensive pharmacy services to patients, including drug therapy assessment based upon evaluation and interpretation of patient history, age, medical condition, surgical history and objective physical biochemical data; development of therapeutic goals and endpoints of drug regimen; preparation of discharge orders and patient education within scope of practice.
Manage drug therapy protocols as a part of an interdisciplinary team. Collaborates with multidisciplinary teams to assist with problem identification and resolution, cost containment issues, implementation of new services, and system/performance measures.
Prospective involvement in medication therapy decisions including collaborative drug therapy practice. Reviews, interprets and evaluates medication therapy to identify, prevent and resolve potential or actual drug-related problems.
Develop, implement and evaluate programs, or area services and initiatives. Leads development of practice standards and measurable outcomes based on performance evaluation and research. Collaborates on investigational drug studies, as necessary.
Actively participate in daily rounds with physicians to identify patients for therapeutic monitoring and to discuss potential interventions.
Monitor drug therapy and suggests alternate drug therapy when appropriate.
Coordinate ongoing quality assessments and improvements of clinical pharmacy activities in collaboration with physicians and implements necessary action as appropriate.
Effectively communicate verbally and/or in writing pharmacy functions to the interdisciplinary team.
Serve as a drug information resource person to physicians, patients and other health care staff. Promotes evidence based therapeutics. Documents interventions and their outcomes for use in drug therapy quality improvement efforts.
Prepare clear and concise clinical briefs for use by nurses, medical staff and pharmacy.
Act as a liaison and consults with nursing and medical staff concerning prescribing, transcribing, dispensing, administration/monitoring of medications; reports patient adverse drug reactions, medication variances and complaints; participates in educational activities and programs for the interdisciplinary team.
Supervise employees directly to ensure safe, legal, and effective drug distribution and use.
Participate in QA review; regularly documents inspection of all medication storage areas.
Check stock on regular basis to identify and reorder outdated stock and ensure that stock is maintained in accordance with manufacturer requirements.
Ensure secure storage of narcotics and other controlled substances on a regular basis and verifies inventory control.
Interpret and clarify orders for all types of medications.
Ensure delivery of quality pharmacy services for inpatients, outpatients, and employees in accordance with established legal requirements.
Compound, label, and package medications and pharmaceuticals meeting professional standards and guidelines.
Maintain and enhance professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops; demonstrates a comprehensive clinical pharmacy knowledge base.
Attend meetings and participates on committees as directed/required.
Serve as a preceptor to pharmacy residents and students when applicable
Required qualifications for this position include:
Graduate from an accredited school or college of pharmacy.
Meets the Pharmacy Department-specified competency requirements consistent with a post-graduate year 1 residency (PGY1) or post-graduate year 2 residency (PGY2) or Has completed a GPY1 residency, or has a minimum of 2 years of experience in area of specialization in lieu of residency.
Current California Registered Pharmacist License in good standing
Current LA City Fire Card within 90 days of hire
A post-graduate year 1 residency (PGY1), or A minimum of 2 years of experience in area of specialization.
Preferred qualifications for this position include:
Post-graduate year 2 residency (PGY2) or Minimum of 3 years of experience in area of specialization.
Board Certified Pharmacotherapy Specialist (BCPS), or appropriate Pharmacy Specialties (BPS) certification, within 2 years of hire in area of specialization.
