Providence Health & Services Clinical Pharmacist Managed Care in Mission Hills, California
Providence is calling a Clinical Pharmacist Managed Care to our Facey (Full Time/ Day Shift) location in Mission Hills, CA.
In this position you will:
- Serve as an integral part of the Facey team, and work closely with the Physicians, Allied Health Providers, Ambulatory Care Clinics, Disease Management Programs, Clinical Pharmacy programs and initiatives, Transitions of Care Programs, Utilization Management, Rx Refill, Operations and other clinical activities. In collaboration with the Clinical Care Teams, the Managed Care Clinical Pharmacist performs analysis of pharmaceutical trends, participates in pharmaceutical education with patients and providers, recommends therapeutic interchanges, assists with prior authorizations, works with payers, conducts pharmaceutical patient assessment (initial and reassessments), develops and manages the patient's chronic illness pharmaceutical care plan, and coordinates that plan with the health care team. In addition, the Managed Care Clinical Pharmacist is responsible for performing various clinical duties related to utilization management of medications. This includes but not limited to reviewing and rendering coverage determinations based on clinical criteria and health plan(s) division of financial responsibility and pharmacy related policy updates as needed. The Managed Care Clinical Pharmacist will also work closely with Rx refill department leadership and serve as the subject matter expert to develop policy and procedures in compliance with all pharmacy related regulatory agencies and help review guideline criteria for prior authorization and non-formulary drugs ordered by Facey Providers. The Managed Care Clinical Pharmacist will also support service area and regional pharmacy and quality initiatives and programs. In addition, clinical guidance, support and oversight to the assigned Pharmacy Technicians will be provided as needed by the Managed Care Clinical Pharmacist.
Required qualifications for this position include:
Current valid California Pharmacist license required
Current American Heart Association BLS for Healthcare Providers required
Doctorate of Pharmacy from a college or school of pharmacy accredited by ACPE
Minimum (3) three years of pharmacy experience in acute care, ambulatory care or managed care settings
Experience with electronic medical records and electronic prescribing
Knowledge and expertise in clinical pharmacology and disease state management using most up to date evidence based practice
Ability to educate and persuade patients and/or family members to alter behavior in accordance with program goals and objectives
General knowledge and understanding of Accountable Care Organizations, MediCare PartD, HEDIS, CMS Stars, and Value-Based Purchasing
General knowledge and understanding of pharmacoeconomics
Ability to analyze data and formulate sound conclusions and recommendations
Ability to work independently and manage multiple, concurrent projects for completion in a thorough and timely manner
Preferred qualifications for this position include:
Chronic disease state knowledge base and related pharmaceutical counseling and/or motivational interviewing experience
Knowledge of clinical pharmacy practice to include: patient monitoring, literature evaluation, formulary enforcement, drug utilization reviews and drug interactions
Certification in Pharmacotherapy, Ambulatory Care, Diabetes or Geriatrics preferred
